MISSION STATEMENT In the spirit of Mother Clelia Merloni, foundress of the Apostles of the Sacred Heart, the staff and volunteers of Clelian Adult Day Center serve the frail, elderly and disabled adults in a medical, social, spiritual, and health care environment. The program is designed to improve the quality of life for each of its participants, emphasizing the inherent dignity and worth of each member. WHAT IS CLELIAN ADULT DAY CENTER? It is a private, non-profit, nondenominational facility operated by the Apostles of the Sacred Heart of Jesus, established in September 1988 to provide a medical, social and spiritual environment for the elderly, frail, and disabled adults whom it serves. WHERE IS IT LOCATED? It is located at 261 Benham Street in Hamden within Sacred Heart Manor, adjacent to Sacred Heart Academy. The site is a beautiful wooded ridge above the area of the Hamden Plaza. WHAT ARE THE HOURS OF OPERATION? The Center is open Monday through Friday from 7:30 A.M. until 4:30 P.M. People may choose to come any number of days or any number of hours in a given day. WHAT DO PEOPLE DO THERE? Recreational programs are scheduled throughout the day to enhance participants cognitive, physical, social and emotional well-being. Reminiscence, Sensory Stimulation, Creative Therapies in Art and Music, Intergenerational Activities, Seasonal and Religious Celebrations, Pet and Horticultural Therapy, Bingo, Cards, Sports and Outings, these are some of the activities offered. All activities are developed and implemented by a Certified Therapeutic Recreational Specialist and Team. IS THERE TRANSPORTATION? Transportation is provided by the Greater New Haven Transit District, the Town of Hamden and family members. The Clelian Adult Day Center arranges all the rides for the Transit Van and the Hamden Van. The days and times are decided by the family. WHAT SERVICES ARE OFFERED? Nursing Services Physical and Occupational Therapy Podiatry and Hair Care Bathing Dietary Services Therapeutic Recreation Pastoral Care Family Counseling Education and Support for the Caregiver WHO IS ON THE DIRECT CARE STAFF? The Clelian Adult Day Center staff includes a Director/R.N., Assistant Director/Certified Therapeutic Recreational Specialist, Assistant Recreational Therapist, L.P.N., Certified Nurses Assistants and Recreational Aide. The supportive staff includes a Receptionist/Transportation Coordinator, Bookkeeper, Grant Writer, Development Director and Office Personnel. ARE ANY MEALS SERVED AT THE CENTER? Our kitchen staff, consisting of a cook and assistants, provide a continental breakfast, a full home-cooked meal for lunch and a snack in the afternoon. Meals are approved by a Registered Dietician to meet nutrition requirements. Accommodations are made for special dietary needs. IS THERE A BASIC FEE? There is an established daily fee. Other sources of payment are available through special programs and grants. Individualized arrangements can be made for your needs. |
